  4. Are there certain things I need to include in medical record documentation for intractable pain patients?

Your documentation should include:

▪ History, physical findings, and results of imaging and laboratory studies relevant to the pain


▪ Results of pain management strategies already used by the patient and their effectiveness and tolerance – in sufficient detail to justify the program’s definition of intractable pain,

▪ The patient’s comprehensive pain management plan going forward,

▪ And the patient’s treatment course after he or she started using medical cannabis.

The MN Department of Health has the authority to request medical records for several purposes, including validating compliance with program requirements.
