FIND A HEALTH CARE PRACTITIONER:
Patients need to have a qualifying condition and register with Minnesota’s Department of Health, Office of Medical Cannabis. Any Minnesota licensed physician, physician assistant (PA) or an advanced practice registered nurse (APRN) can certify patients if they choose. You will need to certify your qualifying condition with a Physician registered in the Minnesota State Medical Cannabis Program/ Feel free to use our list to begin your search.
* This evaluation is required by the state to be filled out before any dispensation of products—including refills. In case you forget, every Green Goods Minnesota location has a computer terminal to complete your evaluation on-site.

Caregivers and Guests of the patients will need to bring their State Photo ID.
